Course Description:
By completing this course you will become familiar with the unique challenges and the approaches used to restore independent functioning of people when they return home from a hospital stay.

- Identify the principles of an effective reablement care model in practice.
- Recognise the benefits of a reablement model.

This course is appropriate for direct care workers, coordinators and case managers working in home care.
